Job summary

Prairie Vista Village is seeking a dedicated LPN Charge Nurse to join our team in Altoona, IA. The LPN will plan, organize, and assist in providing direct and indirect resident care following established policies and standards of practice.

Orients and supervises staff while ensuring compliance with Federal, State, and company requirements. We offer 8-hour shifts, full-time and part-time options, with benefits including health, dental, and vision care, a 401(k) with company match, PTO, and career

Qualifications

• Graduate from an accredited school and hold a current LPN license.
• Currently CPR certified and licensed in the applicable state.
• 1–2 years of prior nursing experience as a Charge Nurse in Long Term Care or Skilled Nursing Facility preferred.

Responsibilities

• Make daily rounds to assess resident condition and comfort.
• Initiate procedures to protect residents from injury and infection.
• Assist DON and/or RN with plans of care and evaluations.
• Oversee staff in implementing care plans and nursing policies.
• Communicate with Physicians and ensure hand-off with oncoming shift.
• Supervise direct care staff and ensure tasks are completed as assigned.
