Qualifications

• Current LPN license in NY is required.
• Experience in geriatric care or skilled nursing setting preferred.
• Ability to evaluate and treat independently with limited supervision.

Responsibilities

• Provide medication administration and IVs.
• Take and record vital signs.
• Provide resident care and treatment with outcome monitoring.
• Educate residents and families on care plans.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
