Apple Rehab West Haven is seeking a licensed practical nurse (LPN) charge nurse to join our caring team at 308 Savin Avenue in West Haven. The role supports nursing services, medication administration, and resident care planning within a 90-bed facility.

The position offers competitive hourly pay based on experience, with shifts 7a-3p, 3p-11p, and 11p-7a. Per diem staff may apply; weekend on-call required. Beneficiary options are available for eligible staff.

Qualifications

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).
• Experience in long-term care is preferred.
• Strong attention to detail and organizational skills.

Responsibilities

The LPN Charge Nurse plays a pivotal role in planning, executing, and overseeing nursing services for assigned residents. This includes adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions.

Key Duties

• Efficiently plans and provides professional care while aligning with the needs of residents and the changing medical landscape.


• Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.


• Documents crucial information related to resident care accurately and comprehensively.


• Administers prescribed medications and oversees accurate record-keeping in line with facility policies.

Requirements

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).


• In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.


• Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively.


• Collaborative team player with previous experience in long-term care preferred.

Additional Expectations

All LPN positions require weekend on-call availability to support both staff and residents. Compensation details available upon inquiry. Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.
