Job summary

Rockville Center is seeking a Licensed Practical Nurse (LPN) for our Skilled Nursing/Long Term Care units. You will monitor residents, administer medications (including IVs), and record vital signs while coordinating with physicians and families.

The role involves supervising CNAs, providing education to residents and families, and working with a dedicated geriatric care team. Shifts are 7AM-3PM with every other weekend required.

Qualifications

• Current NY LPN license required.
• Experience in geriatric care preferred.
• Alternate weekends required.

Responsibilities

• Provide care for residents in SNF/Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs; monitor outcomes.
• Provide resident and family education; communicate with physicians and CNAs.
• Supervise and support CNAs.

Rockville Center is searching for a talented and motivated Licensed Practical Nurse (LPN) to join our dynamic team.

The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
