Job summary

Fairport Rehab and Nursing Center is seeking a compassionate LPN to join our long-term care team. You will deliver direct nursing care, administer medications, and work closely with RNs, CNAs, and other health professionals to support residents’ dignity and quality of life.

Licensed Practical Nurse (LPN) license in NY required; FT benefits include a $3,000 sign-on bonus and competitive pay with weekday/evening/night differentials. Options for full-time or part-time shifts are available.

Qualifications

• Current and valid New York State Licensed Practical Nurse (LPN) license.
• Experience in Long-Term Care, Skilled Nursing, or Rehabilitation preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.
• Compassionate attitude and genuine interest in geriatric care.

Responsibilities

• Administer medications and treatments per physician orders and policies.
• Provide direct nursing care to residents per standards of practice.
• Monitor health status, report changes promptly to clinicians.
• Perform nursing assessments and contribute to care plans.
• Supervise CNAs to ensure quality resident care.
• Respond to emergencies and changes in condition appropriately.
• Maintain accurate documentation of care, meds, and treatments.
• Communicate with residents, families, and interdisciplinary team.

About The Position

As an LPN in our Long-Term Care facility, you will provide direct nursing care to residents while working collaboratively with Registered Nurses, Certified Nursing Assistants, physicians, and other healthcare professionals.

Key Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policies.
• Provide direct nursing care to residents in accordance with established standards of practice.
• Monitor residents' conditions, assess changes in health status, and promptly report concerns to the appropriate healthcare professionals.
• Perform nursing assessments and contribute to individualized resident care plans.
• Supervise and provide direction to Certified Nursing Assistants to ensure quality resident care.
• Respond to resident needs, emergencies, and changes in condition appropriately.
• Maintain accurate and timely documentation of nursing care, medications, and treatments.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Promote infection control, safety, and a clean, comfortable environment.
• Provide compassionate, respectful, and person-centered care to all residents.

Compensation & Schedule

• Pay Range: $30.00 – $34.00 per hour, commensurate with certification and years of post-licensure experience.
• Sign-On Bonus: $3,000 for Full-Time LPNs.
• Weekday Shift Differentials: Additional $1.50/hour for evening and overnight shifts.
• Weekend Differential: Additional 10% per hour for all weekend shifts.

Available Shifts

• Day Shift: 7:00 AM – 3:00 PM
• Evening Shift: 3:00 PM – 11:00 PM
• Night Shift: 11:00 PM – 7:00 AM

Employment Opportunities

• Full-Time: 37.5 hours per week
• Part-Time: 22.5 hours per week
• Weekend availability required.
