Fairport Rehab & Nursing Center is seeking compassionate LPNs to join our long-term care team. You will provide direct nursing care to residents, administer medications, and collaborate with RNs, CNAs, physicians, and other professionals to deliver high-quality, patient-centered care in a supportive environment.

Competenze

• Current and valid New York State LPN license.
• Experience in Long-Term Care, Skilled Nursing, or Rehab preferred.

About the Position

As an LPN in our Long-Term Care facility, you will provide direct nursing care to residents while working collaboratively with Registered Nurses, Certified Nursing Assistants, physicians, and other healthcare professionals.

Key Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policies.

• Provide direct nursing care to residents in accordance with established standards of practice.

• Monitor residents' conditions, assess changes in health status, and promptly report concerns to the appropriate healthcare professionals.

• Perform nursing assessments and contribute to individualized resident care plans.

• Supervise and provide direction to Certified Nursing Assistants to ensure quality resident care.

• Respond to resident needs, emergencies, and changes in condition appropriately.

• Maintain accurate and timely documentation of nursing care, medications, and treatments.

• Communicate effectively with residents, families, physicians, and interdisciplinary team members.

• Promote infection control, safety, and a clean, comfortable environment.

• Provide compassionate, respectful, and person-centered care to all residents.

Qualifications

• Current and valid New York State Licensed Practical Nurse (LPN) license.

• Previous experience in Long-Term Care, Skilled Nursing, or Rehabilitation preferred.

• Strong clinical, communication, and organizational skills.

• Ability to work independently and as part of an interdisciplinary team.

• Compassionate attitude and genuine interest in working with the geriatric population.

• Ability to recognize and respond appropriately to changes in residents' conditions.

Compensation & Schedule

• Pay Range: $30.00 – $34.00 per hour, commensurate with certification and years of post-licensure experience.

• Sign-On Bonus: $3,000 for Full-Time LPNs.

• Weekday Shift Differentials: Additional $1.50/hour for evening and overnight shifts.

• Weekend Differential: Additional 10% per hour for all weekend shifts.

Available Shifts: Full time and Part time available

• Day Shift: 7:00 AM – 3:00 PM

• Evening Shift: 3:00 PM – 11:00 PM

• Night Shift: 11:00 PM – 7:00 AM

Employment Opportunities:

• Full-Time: 37.5 hours per week

• Part-Time: 22.5 hours per week

• Weekend availability required.
