The Phoenix Rehabilitation and Nursing Center is hiring a full-time LPN to join our skilled nursing facility team. The role involves direct patient care for geriatric residents, medication administration, vital signs, and collaboration with physicians and CNAs.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Shifts available: Day shift , Evening shift, Night shift
