The Phoenix Rehabilitation and Nursing Center in Town of Concord, NY is hiring a full-time Licensed Practical Nurse (LPN) to join our skilled nursing team. The role focuses on geriatric resident care, medication administration, vital signs, and education, with opportunities to perform functional therapeutic activities to improve resident experience.

Shifts include day, evening, and night, with a $10,000 Sign-On Bonus and daily pay options. Immediate hiring is available for qualified NY LPNs.

Qualifications

• Must hold NY LPN license.
• Demonstrated ability to provide resident-focused care in a skilled nursing setting.

Responsibilities

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcome.
• Provide resident and family education and communicate with physicians and other health care professionals.
• Supervise CNAs.
