Stellar Senior Living's Mantey Heights Rehabilitation & Care Center in Grand Junction, Colorado is seeking a Licensed Practical Nurse (LPN) for part-time or PRN opportunities. Pay ranges from $29.50 to $38.52 per hour depending on experience.

You will provide direct nursing care, administer medications, monitor residents, and supervise CNAs while collaborating with the care team. Flexible PRN scheduling is available, with requirements including a Colorado LPN license, CPR, and active nursing

Qualifications

• Current Colorado LPN License.
• Graduate of an accredited practical nursing program.
• Strong clinical, organizational, and communication skills.
• Long-term care or skilled nursing experience preferred but not required.
• Current CPR certification or ability to obtain upon hire.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Administer medications and treatments safely and accurately.
• Monitor resident conditions and report changes to physicians and nursing leadership.
• Complete assessments and documentation per facility policies and state and federal regulations.
• Supervise and support CNAs in delivering resident care.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Promote resident dignity, independence, and quality of life.

Position Details

Grand Junction, Colorado

Pay Range: $29.50 - $38.52 per hour DOE

PRN Schedule

Flexible scheduling available based on facility needs.

PRN Commitment Requirements

• Must work a minimum of 2 shifts per month
• At least 1 shift per month must be worked on a weekend
• Must maintain active employment status and complete all required training's and competencies
