Job summary

Stellar Senior Living in Grand Junction, CO is seeking a Licensed Practical Nurse (LPN) for Part-Time & PRN opportunities at Mantey Heights Rehabilitation & Care Center. You will provide direct nursing care, administer medications, and collaborate with a multidisciplinary team to promote resident dignity and quality of life.

The role requires a Colorado LPN license, good clinical and communication skills, and a commitment to compliant documentation.

Qualifications

• Provide direct nursing care to residents per physician orders and care plans.
• Administer medications and treatments safely and accurately.
• Monitor resident conditions and report changes to physicians/nursing leadership.
• Complete assessments and documentation per policies and regulations.
• Supervise and support CNAs in delivering resident care.

Position Details

Grand Junction, Colorado

Pay Range: $29.50 - $38.52 per hour DOE

PRN Schedule

Flexible scheduling available based on facility needs.

PRN Commitment Requirements:

• Must work a minimum of 2 shifts per month
• At least 1 shift per month must be worked on a weekend
• Must maintain active employment status and complete all required training's and competencies

What You'll Do

• Provide direct nursing care to residents in accordance with physician orders and individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor resident conditions and report changes to physicians and nursing leadership.
• Complete assessments and documentation in accordance with facility policies and state and federal regulations.

Qualifications

• Current Colorado LPN License
• Graduate of an accredited practical nursing program
• Strong clinical, organizational, and communication skills
• Long-term care or skilled nursing experience preferred but not required
• Current CPR certification or ability to obtain upon hire
