Job summary

HopeWest is seeking a compassionate Licensed Practical Nurse (LPN) to join our PACE team in Grand Junction, CO. This regular full-time role involves hands-on home-based nursing, medication administration, and coordination with an interdisciplinary team to support aging adults in the community.

The LPN will monitor participants, document care, and educate families while maintaining a patient-centered approach and compliance with care plans and regulations.

Qualifications

  • Current and valid LPN license in the State of Colorado.
  • Graduate of an accredited practical nursing program.
  • Current Colorado driver’s license and auto insurance.
  • Previous nursing experience preferred; working with older adults is a plus.
  • Strong clinical judgment and communication skills.
  • Collaborative, flexible approach to patient care.
  • Genuine commitment to compassionate, person-centered care.

Responsibilities

  • Provide direct nursing care to PACE participants in home settings according to care plans and orders.
  • Monitor health status and report changes to the clinical team.
  • Administer medications and treatments within LPN scope.
  • Collaborate with RNs, providers, CNAs, therapists, social workers and other team members.
  • Support participant education, independence, safety and well-being.
  • Maintain accurate clinical documentation and follow regulations and standards.

Schedule: 3x12 hour shifts

At HopeWest, we believe exceptional care begins with exceptional people. We are looking for a compassionate, skilled Licensed Practical Nurse (LPN) to join our growing PACE (Program of All-Inclusive Care for the Elderly) team.

PACE provides comprehensive, coordinated healthcare and supportive services that allow older adults with complex medical needs to remain living independently in their homes and communities for as long as possible. As an LPN, you will play an important role in delivering hands-on care, supporting participants and families, and collaborating with an interdisciplinary team committed to providing person-centered care.

What You'll Do

As a PACE LPN, you will provide direct nursing care and clinical support to participants in their homes. Responsibilities may include:

Provide direct nursing care to PACE participants in accordance with individualized care plans and provider orders.

Monitor participant health status and promptly communicate changes in condition to the appropriate clinical team members.

Administer medications and treatments within the scope of LPN practice.

Collaborate closely with RNs, providers, CNAs, therapists, social workers, and other members of the interdisciplinary team.

Support participant education and encourage independence, safety, and overall well-being.

Help ensure a welcoming, compassionate, and participant-centered experience.

Maintain accurate and timely clinical documentation.

Follow all applicable regulations, policies, procedures, and standards of care.

What You Bring

Current and valid Licensed Practical Nurse (LPN) license in the State of Colorado, maintained throughout employment.

Graduate of an accredited practical nursing program.

Current and valid Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation.

Previous nursing experience preferred; experience working with older adults or individuals with complex medical needs is a plus.

Strong clinical judgment and the ability to recognize and communicate changes in participant condition.

Excellent communication and interpersonal skills.

A collaborative, flexible approach to patient care.

A genuine commitment to compassionate, person-centered care.
