Job summary

Mantey Heights Rehabilitation & Care Center in Grand Junction, Colorado is seeking a Licensed Practical Nurse (LPN) for part-time and PRN opportunities. You will deliver direct nursing care, administer medications, and support residents according to physician orders and care plans.

This role requires a current Colorado LPN license, strong clinical and communication skills, and may involve shift differentials.

Qualifications

• Current Colorado LPN License.
• Graduate of an accredited practical nursing program.
• Strong clinical, organizational, and communication skills.
• Long-term care or skilled nursing experience preferred but not required.
• Current CPR certification or ability to obtain upon hire.

Responsibilities

• Provide direct nursing care to residents in accordance with physician orders and individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor resident conditions and report changes to physicians and nursing leadership.
• Complete assessments and documentation in accordance with facility policies and state and federal regulations.
• Supervise and support CNAs in delivering exceptional resident care.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Promote resident dignity, independence, and quality of life.

Position Details

Grand Junction, Colorado

Pay Range: $29.50 - $38.52 per hour DOE

PRN Schedule

Flexible scheduling available based on facility needs.

PRN Commitment Requirements:

• Must work a minimum of 2 shifts per month
• At least 1 shift per month must be worked on a weekend
• Must maintain active employment status and complete all required training's and competencies

What You'll Do
