Creekside Center for Rehabilitation and Healing is seeking an LPN to join our post-acute care team in Goodlettsville, TN. The role focuses on medication administration, patient care, and collaboration with the interdisciplinary team to support rehabilitation goals.

The position offers top pay, weekly and daily pay options, and shifts including day, night, full-time, part-time, and per diem. Eligible for benefits, with opportunities to contribute to a caring, community-focused facility.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care services by charting in resident medical records and department records.
• Educate residents and families about treatment and rehabilitation.
• Adhere to infection control policies and facility policies.
• Consult with resident’s physician regarding plan of care and changes.
• Maintain confidentiality and privacy of residents.
• Ensure quality of care per DOH and facility standards.
• Collaborate with interdisciplinary team to solve resident issues.

Salary: Up to $33 an hour

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
