Job summary

Creekside Center for Rehabilitation and Healing, located in Tennessee, is seeking a Licensed Practical Nurse (LPN) to join our post-acute care team. We offer top pay with weekly pay and NOW OFFERING DAILY PAY, with shifts including day, night, full time, part time, and per diem.

The LPN will administer medications, assist with admissions and discharges, document patient care, educate residents and families, and collaborate with physicians and the interdisciplinary team to ensure high-quality

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents.
• Document resident care services by charting in resident medical records.
• Educate residents regarding treatment, rehabilitation, change of status, and inform family members.
• Adhere to infection control policies and facility policies and procedures.
• Consult with resident’s physician regarding plan of care and notify of changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care by adhering to DOH standards and facility standards of care.
• Assist with solving resident problems by collaborating with the interdisciplinary team.

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
