Job summary

Creekside Center for Rehabilitation and Healing in Mount Juliet, TN is hiring an LPN to support post-acute care across physical, occupational and speech therapies. You will administer medications, assist with admissions/transfers, and document care in resident records.

We offer up to $33/hour with weekly and daily pay options, plus benefits such as insurance and discounts. Shifts include day, night, full-time, part-time, and per diem.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
• Experience providing patient care and medication administration preferred.
• Ability to educate residents and family members about treatment plans.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care in medical records and department files.
• Educate residents and families about treatment and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with physicians regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Collaborate with interdisciplinary team to ensure quality of care per DOH standards.

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
