Job summary

Creekside Center for Rehabilitation and Healing is seeking an LPN to provide post-acute care across physical, occupational and speech therapy programs. The facility emphasizes high standards of care and ongoing innovation, with immediate hiring and flexible shifts.

The role includes medication administration, charting, resident education, and collaboration with an interdisciplinary team to ensure DOH and facility standards are met. Compensation includes up to $33/hour and various benefits.

Qualifications

• Must possess a nursing degree from an accredited college or university or be a graduate of an approved LPN or RN program.
• Licensure as LPN or RN in good standing.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document care in resident records and department records.
• Educate residents and families about treatment and status changes.
• Adhere to infection control policies and procedures.
• Consult with physician regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Coordinate with interdisciplinary team to ensure quality care.

Salary: Up to $33 an hour

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:
