Creekside Center for Rehabilitation and Healing in Tennessee seeks an LPN to deliver post-acute care, including medication administration and resident education, under supervision of RNs and physicians.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
• Ability to administer medications and provide resident care within post-acute therapy programs.
• Maintain accurate documentation in resident medical records and department records.

Responsibilities

• Administer medications and treatments to residents.
• Assist with admission, transfer and discharge processes.
• Document care in medical records and department systems.
• Educate residents and families about treatment plans and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with physicians regarding plans of care and changes.
• Ensure resident confidentiality and privacy.
• Support interdisciplinary team to resolve resident issues.

Salary: Up to $33 an hour

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
