Job summary

The Grand Rehabilitation and Nursing at Queens in New York is seeking a dedicated LPN to join our Skilled Nursing facility team. You will provide hands-on resident care, monitor health status, administer medications, and collaborate with physicians and CNAs to support optimal outcomes.

Full-time and part-time positions are available. An active NY LPN license is required; shifts include 7am-3pm and 3pm-11pm; daily pay offered. Competitive wages and benefits accompany this role. INDLPN

Qualifications

• Current LPN license in New York state.
• Experience in geriatric/long-term care settings preferred.
• Capable of independent assessment and basic treatments.
• Ability to educate residents and families about care plans.

Responsibilities

• Administer medications, including IVs where trained.
• Take and record vital signs and monitor outcomes.
• Provide resident care and functional therapies to improve well-being.
• Educate residents and families; communicate with physicians and CNAs.
• Supervise CNAs and coordinate care with the health team.

Rate: Up to $34.48 an hour

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
