Job summary

The Phoenix Rehabilitation and Nursing Center is seeking a full-time Licensed Practical Nurse (LPN) to join our skilled nursing facility team in New York. The candidate should be passionate about geriatric care and able to evaluate and treat independently with supervision, providing functional therapies to enhance residents’ experiences.

We offer a Sign-On Bonus of up to $10,000 and daily pay options, with shifts available across day, evening, and night.

Qualifications

• Current Licensed Practical Nurse (LPN) license in NY.

Responsibilities

• Provide care for residents in SNF and Long Term Care Unit.
• Medication administration including IVs.
• Take and record vital signs.
• Provide resident/family education and care with monitoring of outcomes.
• Communicate with physicians, NPs, families and other health professionals.
• Supervise CNAs.
