Job summary

Complete Care at Silver Lake is seeking an LPN Licensed Practical Nurse (Direct Care) to join our team in Dover, DE. We offer a $7,500 sign-on bonus for full-time and a $3,750 sign-on bonus for part-time, with FT/PT options and PRN opportunities.

You will administer medications, perform wound care, monitor residents, and collaborate with physicians, therapists, and social workers to deliver person-centered care. Our program supports career growth, benefits, and a balanced work life.

Qualifications

• LPN license required and valid in Delaware.
• Experience in long-term care or hospital discharge settings preferred.
• Ability to lead and collaborate with care teams.

Responsibilities

• Administer medications (oral, topical, IV, injectable).
• Perform wound care and dressings; manage pain.
• Monitor residents' health and track changes.
• Provide catheter/ostomy care and collect specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists and social workers.
• Educate families and residents on health status and care plans.
• Maintain detailed health records and care interactions.
• Oversee and delegate tasks to CNAs to ensure quality care.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
