We are seeking a skilled and dedicated LPN (Licensed Practical Nurse) to join our team of healthcare professionals. As an LPN at Valley Manor, you will play a vital role in delivering high-quality patient care and support to our residents. Your expertise and commitment will enable us to provide a safe and dignified environment for our patients to receive the care they deserve.
Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
Conduct thorough patient assessments and develop individualized care plans in collaboration with the healthcare team
Implement and monitor treatment plans, ensuring timely and effective patient outcomes
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in patient education and support, promoting independence and empowerment
Maintain accurate and timely records, adhering to facility and regulatory standards
Current LPN licensure in the state of Pennsylvania
Ability to work varied shifts, including days, evenings, and weekends
Strong communication and interpersonal skills
