• Develop and implement individualized care plans that address the unique needs and goals of each patient.
• Collaborate with the interdisciplinary care team to ensure comprehensive care and coordination of services.
• Document patient care and progress in accordance with facility policies and regulations.
• Participate in quality improvement initiatives and contribute to the development of best practices.
