Signature HealthCARE of Glasgow Rehab & Wellness Center is seeking an LPN/LVN to provide direct nursing care, supervise aides, and maintain high-quality resident care. The role emphasizes safety, documentation, and collaboration with an interdisciplinary team in a 78-bed facility.

The position requires an active LPN/LVN license in Kentucky, with preference for supervisory experience, LTC familiarity, and CPR/BLS certification. Proficiency in EHR systems and MS Office is desired.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents’ condition and notify physician of changes.
• Develop, implement, and review resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges following procedures.
• Participate in quality assurance activities and regulatory surveys.
