Job summary

Signature HealthCARE in Kentucky seeks a dedicated LPN/LVN to provide direct nursing care to residents, administer medications, and support an interdisciplinary team in a 97-bed facility committed to resident-centered care and quality of life.

You will supervise nursing assistants and aides, coordinate admissions and discharges, document accurately in the electronic health record, and ensure safety, dignity, and regulatory compliance at all times.

Qualifications

• Holds an active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; LTC experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong verbal and written communication skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Document accurately in the electronic health record per policy.
