Job summary

Signature HealthCARE is seeking an LPN/LVN to provide direct nursing care at our Kentucky facility. The role includes supervising nursing staff, administering medications, and coordinating resident care plans within a multidisciplinary team.

Qualified candidates will hold an active LPN/LVN license in Kentucky with at least one year of clinical nursing experience, preferably in long-term care, and familiarity with EHR systems. We offer comprehensive benefits and a supportive workplace culture.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
