Job summary

Signature HealthCARE LLC is seeking a dedicated LPN/LVN to provide direct nursing care to residents at our Kentucky facility. The role includes medication administration, wound care, and ADL support within a supportive, team-based environment.

Ideal candidates hold an active LPN/LVN license, have 1+ year of nursing experience (long-term care preferred), and possess strong clinical observation and documentation skills. EHR familiarity and good communication are essential.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Supervisory/charge nurse experience preferred.
• Experience with EHR systems and strong documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Excellent verbal and written communication abilities.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise and direct nursing staff during shifts.
• Monitor residents' condition and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation.
• Coordinate admissions, transfers, and discharges.
