Position Summary

The LPN at Trinity Health Kenmare provides direct, resident-centered nursing care under the supervision of a registered nurse, supporting both long-term care and rehabilitation services. This role assists with resident assessments, contributes to individualized care planning, and delivers delegated nursing interventions while monitoring for changes in condition. The LPN works collaboratively with providers, families, and the interdisciplinary care team to support continuity, safety, and quality of care in a structured care environment. Through consistent clinical practice, compassionate resident interaction, clear communication, and adherence to regulatory and clinical standards, this position promotes resident well-being, functional progress, and positive care outcomes.

Key Responsibilities
  • Assist the RN with assessments, data collection, and ongoing evaluation of residents’ physical, emotional, and functional status.
  • Provide delegated nursing care including medication administration, basic treatments, wound care, and monitoring of chronic conditions in accordance with established care plans.
  • Document all observations, interventions, and resident responses accurately in the medical record to support coordinated and regulatory compliant care.
  • Communicate promptly with the RN, provider, and family regarding changes in resident condition, and support updates to individualized care plans.
  • Supervise and direct CNA staff during daily care activities, reinforcing proper techniques, safety practices, and resident-centered approaches.
  • Contribute to a clean, safe, and supportive living environment by assisting with equipment readiness, infection prevention measures, and collaboration with support departments.
Licenses and Certifications Required
  • Current or temporary LPN license in ND or compact LPN licensure.
  • BLS certification is required within the orientation period.
Educational Requirements
  • LPN program as evidenced by licensure.
Experience Requirements
  • Previous nursing experience preferred, but not required.
Special Skills or Training Requirements
  • Good organizational and interpersonal skills.
Physical Requirements

The LPN frequently stands, walks, bends, and assists residents with mobility, transfers, and activities of daily living. The role requires the ability to lift, push, or pull equipment or supplies and support residents who may have limited strength or balance. Adequate vision, hearing, manual dexterity, and communication skills are essential for performing clinical tasks, monitoring conditions, and ensuring resident safety.

Environmental Requirements

Work occurs in a longterm care and rehabilitation environment with routine exposure to residents with chronic conditions, changing behaviors, or mobility limitations. The role may involve contact with blood, body fluids, cleaning agents, and medical equipment. The LPN must follow infection prevention protocols, safety procedures, and resident rights standards while adapting to variable workloads, emergencies, and family interactions.
