Position Summary:

The RN, Long Term Care provides comprehensive nursing care to residents in a long-term care setting, supporting both chronic health management and daily living needs. This role conducts ongoing assessments, monitors changes in resident condition, and coordinates individualized care plans that promote comfort, dignity, and quality of life. Working collaboratively with physicians, families, and interdisciplinary team members, the RN helps ensure timely communication, safe care transitions, and adherence to regulatory and organizational standards. As part of a supportive, team-oriented care environment, this position provides clinical leadership and oversight while fostering a stable, resident-centered setting through compassionate, consistent nursing care.

Physical Requirements

The RN, Long Term Care frequently stands, walks, and assists residents with transfers, ambulation, and activities of daily living. Environmental Requirements

This role is performed in a long-term care environment with routine exposure to residents with chronic illnesses, cognitive impairments, and varying levels of mobility.
