Qualifications

Administrative, customer service or healthcare experience required.
Must have an active LPN license in Tennessee; multi-state licensure preferred.
Strong data entry and documentation skills with attention to detail.

Responsibilities

Receives referrals for Home Health/Hospice services and verifies patient information.
Reviews clinical documentation and identifies patients for admission.
Communicates with hospital Case Management to determine needs.
Assists patients and families, physicians, and referral sources as needed.
Ensures integrity of patient data and statistics.
Prepares correspondence and materials for Case Coordinators.
Performs other duties as assigned.
