Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator in Denison, IA to manage referral intake and coordinate care with the interdisciplinary team. You will process referrals through Home Care Home Base and ensure smooth communication with referral sources.

Requirements include an active LPN license, a valid driver’s license, and at least 1 year of LPN experience (home health preferred). Hepatitis profile proof is required; sales/marketing experience is a plus.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• Facilitate the intake referral process and coordinate care with the interdisciplinary team and referral source.
• Process all aspects of referral intake via Home Care Home Base software system.
