Job summary

Angels Care Home Health seeks a licensed practical nurse (LPN) Intake Coordinator to manage referral intake and coordinate care with the interdisciplinary team. You will process referrals using the Home Care Home Base system and ensure timely onboarding of patients.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Job Functions:
