Job summary

HarmonyCares seeks a Patient Health Coordinator to support Medical Group patients through care coordination and progress monitoring. You’ll work with the Integrated Care Team to implement patient-centered plans and guide families through the healthcare journey.

The role emphasizes critical thinking, problem-solving, and strong communication. Expect collaboration with providers, support staff, and external partners in a high-quality care environment.

Qualifications

• High school diploma or GED required.
• 2+ years of care coordination or healthcare experience.
• Familiarity with Medicare, Medicare Advantage and Medicaid insurances.
• Must be organized, detail-oriented, and able to multi-task.
• Strong interpersonal communication and active listening skills.
• Able to think critically and make sound judgments.
• Strong time management and patient advocacy.
• Knowledge of basic medical terminology.

Responsibilities

• Assist with care coordination for paneled patients.
• Collaborate with cross-functional teams to achieve objectives.
• Coordinate in-market activities: relaying messages, referrals, faxes, and prior authorizations.
• Monitor and evaluate interventions and adjust care plans as needed.
• Advocate for patients and participate in High-Risk Huddles.
• Reach out to discharged patients to close care gaps.
• Document all patient interactions promptly in the EHR.
• Communicate clinical information to patients, caregivers, and facilities.
• Work with the care team to ensure quality and patient experience outcomes.
• Identify and address Social Determinants of Health barriers.
