Job summary

Volunteers of America Southeast, Inc in Tuscaloosa, AL is seeking a Licensed Practical Nurse (LPN) to join the IDD program. You will provide direct nursing services, health monitoring, documentation, and staff training in line with ADMH, CMS, and NDP standards.

This full-time role requires clinical nursing experience, strong assessment skills, and the ability to coordinate care with an interdisciplinary team.

Qualifications

• Licensed Practical Nurse (LPN) license in Alabama.
• Successful completion of the ABON Medication Assistance Supervision (MAS) test.
• Minimum 1 year clinical nursing experience, preferably in IDD or community-based care.
• Valid Alabama driver’s license and insurable driving record.
• Experience with electronic documentation systems (Therap).
• Strong communication, organization, and interpersonal skills.
• Ability to train and monitor Medication Assistance Certified (MAC) staff and delegation per NDP.
• Willingness to assume 24-hour on-call duties.

Responsibilities

• Conduct ongoing health assessments and document findings.
• Gather accurate health information to determine referrals or follow-up care.
• Deliver nursing services as prescribed and in compliance with standards.
• Ensure health records comply with regulations and funding requirements.
• Contribute to individualized support planning with medical input.
• Participate in IDT meetings and care plan reviews.
• Schedule medical appointments and coordinate transportation when needed.
• Monitor health service delivery and communicate with medical providers.
• Provide nursing oversight post-hospitalization and follow-up on changes in condition.
• Train direct support staff on medication administration and safety protocols.
• Conduct MAC training, delegation, and oversight per guidelines.
• Maintain knowledge of healthcare resources for IDD populations.
• Willingness to 24-hour on-call nursing duties.
