Job summary

Lowe’s Guardian Angel in Georgia is seeking a part-time Licensed Practical Nurse (LPN) to coordinate medical care and provide clinical oversight for individuals with IDD in residential and day-support settings.

The LPN will complete medical documentation, monitor quality of services, and collaborate with staff and families. Requires current GA LPN license, CPR/BLS, and experience with IDD or related needs. This role emphasizes compliance with DBHDD standards and strong organizational skills.

Qualifications

• Licensed Practical Nurse (LPN) in Georgia with current license.
• CPR/BLS certification required.
• Experience with individuals with IDD, behavioral health needs, or dual diagnoses preferred.
• Knowledge of DBHDD standards, HRST, MAR documentation, medication management, and clinical auditing preferred.
• Strong clinical documentation, communication, organizational, and time‑management skills.
• Ability to work independently while collaborating with program leadership and direct‑care staff.

Responsibilities

• Coordinate medical care and oversee health services for individuals with IDD in residential and day-support settings.
• Complete nursing assessments and monitor health status with timely documentation.
• Attend ISP and SIS meetings as assigned.
• Educate and train staff on medical management and compliance with DBHDD requirements.
• Audit MARs and HRST and ensure proper medication administration and records.
• Coordinate medical appointments and transportation when needed.
• Prepare health reports and participate in quality assurance activities.
