Kaiser Permanente seeks a Licensed Practical Nurse I (LPN I) in Georgia to deliver high-quality nursing care under supervision. The role centers on patients and families within an integrated nursing model, emphasizing coordination, education, and adherence to policy.

The LPN I provides direct and indirect care, reviews patient information, supports diagnostic processes, and collaborates with the health care team to optimize health outcomes.

Qualificações

• Graduate of an accredited LPN/LVN program.
• Practical Nurse License (Georgia) required at hire.
• Basic Life Support (BLS) required at hire.
• Complete a Customer Service Assessment.
• Minimum six months of health care experience preferred.
• Customer service aptitude demonstrated through assessment.

Responsabilidades

• Prepares patient for office visit by reviewing history, collecting vitals, and documenting.
• Completes POE and addresses care gaps during the patient encounter.
• Prepares rooms and assists the practitioner for procedures.
• Initiates interventions and documents responses per guidelines.
• Participates in emergency care as directed.
• Educates patients and provides prep materials for diagnostics.
• Schedules lab, radiology, or specialty appointments.
• Monitors InBasket messages and emails related to requests.
• Participates in quality improvement activities.
• Engages patients and families in care coordination.
• Resolves patient concerns promptly.
• Provides orientation and training to new team members.
• Supervises unlicensed personnel in patient care.
• May perform other duties as assigned.
