Job summary

KAISER in Tucker, Georgia is seeking a Licensed Practical Nurse I (LPN I) to provide direct and indirect nursing care under supervision. You will educate patients, coordinate care, and collaborate with the health care team to meet quality standards and patient needs.

Responsibilities include preparing patients for visits, documenting care, assisting with procedures, and managing communications and referrals. A Georgia LPN license and BLS are required at hire.

Qualifications

• Graduate of an accredited LPN/LVN program.
• Practical Nurse License (Georgia) required at hire.
• Basic Life Support (BLS) required at hire.
• Complete a Customer Service Assessment.
• Minimum six months of healthcare experience preferred.

Responsibilities

• Prepares patient for office visit by reviewing history and collecting relevant clinical information.
• Completes POE and addresses care gaps during the patient encounter.
• Prepares room and equipment for procedure and assists the practitioner as needed.
• Initiates interventions and documents response to treatment.
• Participates in emergency care as directed by practitioner or RN.
• Educates patients and provides preparation materials for diagnostic studies.
• Schedules appointments for patients with lab, radiology, or specialty consultations.
• Monitors InBasket messages related to patient or provider requests.
• Participates in quality improvement activities and initiatives.
• Engages patients, families, and caregivers in coordination of care.
• Resolves patient concerns promptly.
• Provides departmental orientation and training of new team members.
