Corewell Health Hospice in Saint Joseph, MI is seeking a Licensed Practical Nurse (LPN) to join the Hospice West team. The LPN will assess, plan, implement, and evaluate patient care under the direction of an RN, mid-level provider, or physician to support end-of-life services for patients and families.

The role emphasizes interdisciplinary collaboration and compassionate care, with a day shift from 8 a.m. to 8:30 p.m., totaling 36 hours weekly and a weekend rotation, in the Royalton site.

Qualifications

• Must be licensed LPN in Michigan.
• High school diploma or equivalent required.
• Completion of an approved nursing program required.
• Orientation program successfully completed.

Responsibilities

• Assess, plan, implement, and evaluate patient care under supervision.
• Provide end-of-life care and symptom management per care plan.
• Collaborate with RN, mid-level provider, and physician to coordinate care.

Qualifications

Required:

• High School Diploma or equivalent
• Required Completion of an approved nursing program
• Successful completion of competency based orientation program
• License Practical Nursing (LPN) - State of Michigan Upon Hire
• Driver's License - State of Michigan Upon Hire

Preferred

• Associate's Degree or equivalent Graduate of an approved nursing program
• Healthcare experience
