Job summary

Maxim Healthcare is hiring a licensed Practical Nurse (LPN) for private duty home care in Crown Point, IN. You will deliver direct 1:1 nursing care in patients' homes, monitor conditions, and follow individualized care plans with support from local clinical teams.

The role offers stable assignments, flexible scheduling including day/evening/overnight shifts, and opportunities for full-time, part-time, or PRN work. Must have active LPN license and BLS.

Qualifications

• Active LPN license is required.
• Current BLS certification is required.

Responsibilities

• Provide direct patient care in the home.
• Monitor patient condition and report changes.
• Administer medications and treatments as ordered.
• Support individualized care plans.
• Document care and communicate with the healthcare team.
