Job summary

Maxim Healthcare in Omaha is hiring a Licensed Practical Nurse (LPN) to work with pediatric and/or adult patients in their own homes for full-time or part-time day and overnight hours.

The LPN provides direct patient care in the home setting, utilizing the nursing process to plan, implement and evaluate care. The role includes medication administration, care coordination and discharge planning support as required.

Qualifications

• Current Licensed Practical Nurse (LPN) license for the practicing state.
• Current Health Certificate (per facility LPN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year LPN experience preferred.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care and services in the home.
• Implements plan of care and revises it as patient status changes.
• Delivers therapeutic, preventive, and rehabilitative nursing actions.
• Responds to changes in patient condition and informs the physician or supervisor.
• Administers medications and treatments as prescribed and documents effectiveness.
• Reviews care plans and coordinates education and discharge planning as needed.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements

• Current Licensed Practical Nurse (LPN) for the state in which the nurse practices.
• Current Health Certificate (per facility Licensed Practical Nurse contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse (LPN) experience preferred.
