Job summary

Guardian Angel Home Care Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area.

You will provide skilled nursing care in patients' homes, working under the direction of the RN and physician, with a focus on autonomy and meaningful patient relationships. Ideal candidates hold an active Michigan LPN license, have completed an accredited nursing program, and possess BLS/CPR certification.

Qualifications

• Requires active Michigan LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes under RN/physician supervision.
• Monitor patient condition, vital signs, and health status.
• Administer medications and treatments within LPN scope.
• Document visits and care in HCHB accurately.
• Communicate changes to supervising RN/physician.
• Educate patients and families on medications and care plan.
• Ensure compliance with regulations and collaborate with team.

About the Role

Guardian Angel Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing home health team in the Grand Rapids area. In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
• Monitor patient condition, vital signs, and overall health status
• Administer medications and treatments as ordered and within the LPN scope of practice
• Accurately document all visits and care provided in Homecare Homebase (HCHB)
• Communicate changes in patient condition to the supervising RN and physician
• Educate patients and families on medications, disease management, and the plan of care
• Ensure compliance with all federal, state, and agency regulations
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes

Requirements

• Active Michigan Licensed Practical Nurse (LPN) license in good standing
• Graduate of an accredited practical nursing program
• Current BLS/CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical, communication, and documentation skills
• Ability to work independently in a home health setting

Preferred Qualifications

• Home health or hospice experience
• Experience with Homecare Homebase (HCHB) or other electronic medical record systems
• Wound care experience
• Bilingual candidates are encouraged to apply
