Job summary

Corewell Health South Homecare seeks a licensed practical nurse to deliver high-quality in-home nursing care in the Benton Harbor area. The LPN will assess, plan, implement, and evaluate patient care under the Nurse Leader's direction, partnering with patients and families to meet care goals.

The role offers a day shift, 40 hours per week, with weekend coverage on a rotating basis. Comprehensive benefits and a sign-on bonus up to $10,000 are available.

Qualifications

• High School Diploma or equivalent.
• Completion of an approved nursing program.
• LPN in State of Michigan.
• Driver license in State of Michigan.
• BLS within 90 days.
• Graduates from a non-accredited school of nursing must have at least one year of acute care experience or at least two years in a licensed health care role and must have passed the NCLEX exam.
• 1 year of relevant experience in home health or similar clinical setting.

Responsibilities

• Provide professional nursing care using the nursing process.
• Assess, plan, implement, evaluate, and discharge care under ANA scope.
• Collaborate with patient/family to develop and teach plan of care.
