Job summary

UnitedHealth Group's Capital Region Home Health team seeks a Licensed Practical Nurse to deliver high-quality patient care in patients' homes, following the plan of care and infection control standards. You will document visits in the EMR, coordinate care with the health team, and communicate with patients, families, physicians, and case managers to ensure timely updates and optimal outcomes.

The role offers a $5,000 sign-on bonus and compensation that includes per-visit point pay and hourly

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days.
• Home care experience preferred.

Responsibilities

• Provide high quality clinical services within the scope of practice and plan of care.
• Document patient visits in the EMR per regulatory and payer requirements.
• Coordinate with care team and communicate changes in patient condition.
• Work effectively in diverse home settings and with patients of all backgrounds.
