Job summary

Corewell Health South Homecare is hiring an LPN to provide professional nursing care in patients' homes under the ANA nursing process. You will assess, plan, implement, evaluate, and discharge with patient/family collaboration.

You will document education in the health record and support teaching strategies for patients and families. The role requires a Michigan LPN license, a valid driver's license, and BLS within 90 days.

Qualifications

• High School Diploma or equivalent.
• Completion of an approved nursing program.
• LPN in State of Michigan.
• Driver license in State of Michigan.
• BLS within 90 days.
• Graduates from a non-accredited school of nursing must have at least one year of acute care experience or at least two years of experience in a licensed health care role and must have successfully passed the NCLEX licensure examination
• 1 year of relevant experience recent clinical experience in home health and/or other clinical setting

Responsibilities

• The LPN, under the ANA scope of practice utilizes the nursing process to provide professional nursing care.
• Assess, plan, implement, evaluate, and discharge.
• The LPN partners with patient/family input and goals in planning plan of care and develops teaching strategies for patient/family; documents education and learning in health record.

Education
