Assisted Home Health & Hospice in Phoenix seeks a dedicated LPN to join its Home Health Division serving Laveen and surrounding areas. You will implement nursing plans of care and coordinate with the care team.

This role requires an active Arizona LPN license, CPR certification, and at least two years of nursing experience, including hospital or home care supervision. We offer comprehensive benefits and growth opportunities.

Qualificações

• Must hold an active LPN license in Arizona.
• Two years of professional nursing experience required.
• Two years of clinical supervision or charge nurse experience preferred.
• Current CPR certification and health clearance.

Responsabilidades

• Develops and implements a comprehensive plan of care per MD orders with measurable goals.
• Coordinates the plan of care with the care team and other agencies to ensure continuity.

What you will do:

• Develops and implements a comprehensive plan of care including preventative and rehabilitative nursing procedures where appropriate in accordance with MD orders. Identifies problems and establishes measurable goals
• Coordinates the plan of care with members of the care provider team and other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive plan of care

Who you are:

• Current, active LPN License in the State of Arizonaby the Board of Registered Nursing.
• Current CPR, health clearance and clinical skills self-assessment.
• Minimum of two years of professional nursing experience.
• Minimum of two years of experience as a clinical supervisor or charge nurse in a hospital or home care setting.
