Assisted Home Health & Hospice is seeking a Licensed Practical Nurse (LPN) for Hospice East Valley in Phoenix, AZ. This PRN role focuses on providing compassionate, professional end-of-life care using nursing processes to assess, plan, implement, and evaluate patient needs.

The LPN will collaborate with patients, families, and the health care team, maintaining high standards of nursing practice and ensuring comfort and dignity for patients in hospice care.

Qualifications

• Graduate of accredited practical nursing program.
• Current LPN license by the Arizona Board of Nursing.
• Fingerprint card as required for licensure.

Responsibilities

• Deliver holistic, end-of-life care using the nursing process (assessment, planning, intervention, evaluation).
• Interact effectively with patients, caregivers and health team while maintaining professional nursing standards.

What you will do:

As a Hospice LVN you will deliver holistic, end-of-life care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and you will effectively interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.

Who you are:

Graduate of accredited practical (vocational) nursing program. Current, active license as an LPN by the Arizona Board of Nursing including fingerprint card.
