Assisted Home Health & Hospice is seeking a per diem LVN to support our Mission Hills Home Health branch, delivering 10–15 visits per week.

Compensation is $46 per visit plus $11 for mileage. The role requires a California nursing license and at least one year of professional nursing experience, CPR, and agency competency completion.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active nursing license in California.

Responsabilités

What you will do:

As a Home Health LVN you will deliver holistic, home health care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and you will effectively interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.

Who you are:

• Graduate of an approved school of professional nursing.
• Current, active license in the State of California by the Board of Registered Nursing.
• Current CPR, health clearance and clinical skills self-assessment.
• Successful passage (80%) of Agency competency exam and core competency per skills checklist.
• Minimum of one year of professional nursing experience.
