OneWell Health Care in Myrtle Beach, SC is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing services in home and community settings across South Carolina. You will work under RN supervision, administer medications, monitor conditions, and support IDD and other medical needs, promoting safety and independence.

This field-based role requires active SC LPN license, CPR/BLS, TB test, and a willingness to travel to participant homes.

Formación

• Active South Carolina LPN license
• CPR/BLS Certification
• Current TB test or screening
• Experience in home health, IDD services, or long-term care preferred

Responsabilidades

• Deliver physician-directed nursing care by implementing and monitoring individualized care plans.
• Administer prescribed medications and treatments.
• Monitor vital signs and provide wound care.
• Support PEG/G‑Tube or tracheostomy care as applicable and trained.
• Document care and patient status in the EHR.
• Coordinate with families, caregivers, and healthcare providers.
