Job summary

Traditional Home Health Care is seeking a dedicated LPN to join its hospice team in Scranton, PA. You will provide in-home nursing care, follow a physician-approved plan of care, and coordinate with families and the nursing supervisor to ensure quality patient outcomes.

The role requires travel within Scranton area and surrounding communities, strong documentation and communication skills, and the ability to work independently.

Qualifications

• Current license as a Practical Nurse (LPN) in Pennsylvania.
• Must have current driver's license.
• Ability to travel to case assignments.
• Excellent assessment and documentation skills.
• Ability to work independently with minimal supervision.
• Strong interpersonal and communication skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assess patient status and follow plan of care with physician, nursing supervisor, patient, and family.
• Administer patient care and evaluate the plan of care, teaching patients per orders.
• Report status changes to physician and nursing supervisor; revise plan as needed.
• Prepare and document meticulously, including interactions with patients and care changes.
• Act as liaison with patients, therapists, nursing supervisor, and health care team.
• Inform patients about additional services and make referrals as needed.
