Overview

Under the direction of the Clinical Director and/or Registered Nurse, the Licensed Practical Nurse (LPN) provides direct nursing care to patients and performs a variety of indirect patient care duties. The position is responsible for identifying patient/family needs and coordinating their care. As part of the Hospice Interdisciplinary Team (IDT), the LPN contributes to and follows an individualized plan of care, utilizing monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life for the patient/family unit.

Responsibilities

• Deliver direct nursing care to hospice patients.
• Coordinate interdisciplinary care and follow individualized care plans.
• Monitor patient condition, intervene, and educate patients and families.
• Ensure high level of personalized, sincere, and engaging service to patients and families at all times.
• Address patient/family concerns promptly and effectively.

Qualifications

• Graduate of an accredited Licensed Practical Nurse Program.
• Minimum of two years general nursing experience; hospice experience preferred.
• Experience with pain and symptom management interventions preferred.
• Working knowledge of hospice philosophy, principles of death and dying.
• Current licensure as a Practical Nurse in the Commonwealth of Pennsylvania.
• CPR certification obtained and maintained.
• Clearance by PA State Police criminal history check and PA Department of Public Welfare Child Abuse Central Register (Childline).
• Ability to work within an interdisciplinary setting.
• Excellent verbal communication and interpersonal skills.
• High degree of customer awareness and empathy, ability to provide respectful and dignified care.
• Comfortable with manual dexterity for writing, keyboarding, and telephone operation.
• Ability to manage stress and maintain confidentiality of patient information.

Employment Requirements

The LPN functions independently while seeking and accepting guidance from other IDT members and community resources as needed. Professional, ethical conduct and strong problem‑solving skills are essential.

Physical Demands

Medium work: duties require exerting 25‑50 lbs occasionally, 10‑25 lbs frequently, and up to 10 lbs constantly to lift, carry, push, pull, and move objects. Requires bending, reaching, stooping, and lifting throughout the shift; ability to transport self throughout the facility and travel to patient homes if necessary.

Accident or Health Hazards

Occasional exposure to blood and body substances; needle stick exposure possible.

Working Conditions

Works in a state‑of‑the‑art hospice inpatient unit; may also work in patient homes where conditions may be less than desirable.
