Traditional Home Health Care and Hospice is seeking a full-time RN for Scranton, PA and surrounding areas. The role focuses on admissions, care planning, and ongoing patient management in home health settings.

The ideal candidate will hold an active RN license in Pennsylvania, have a valid driver's license, and be able to travel to cases. Strong assessment, documentation, and communication skills are essential.

Qualifications

• Current RN license in Pennsylvania
• Must have a valid driver's license
• Ability to travel to case assignments
• Excellent assessment and documentation skills
• Ability to work independently with minimal supervision
• Strong interpersonal and communication skills
• Acute care and/or home health experience preferred

Responsibilities

• Admits patients to Traditional Home Health Care.
• Assesses patient status and develops plan of care with physician, nursing supervisor, patient and family.
• Administers patient care and manages/educates on the care plan.
• Reports patient status changes to physician and nursing supervisor; revises plan as needed.
• Maintains meticulous documentation including OASIS time points and plan changes.
• Acts as liaison with patients, therapists, nursing staff, and health care team.
• Refers patients to other community resources as needed.
